The document was not preserved, but in general at that time such "certificates" were in use. They replaced current official diplomas. So, on June 8, 1790, Haydn signed a certificate certifying the professional competence of composer and pianist Joseph Eibler. Beethoven himself issued a recommendation to Karl Czerny on May 7, 1805, which stated that he had achieved outstanding success in playing the piano. Such evidence could be useful on tour or in official positions.
